Important Notes Regarding This Request:

• When submitted, this form will be sent to the Jaeckle Customer Service Department for review and processing.
• Jaeckle Distributors will respond within 24 hours with an RA# if this request adheres to Return Policy guidelines.
• After approval, it is the customer’s responsibility to arrange for shipment back to Jaeckle.
• Jaeckle will receive, inspect and issue a credit for the return consistent with the Return Policy.
